Instructions:

1. Complete this form after you have met with your major professor and Renee (first semester students only) and know which courses you will be taking.
2. To fill in all of your personal contact information and class schedule, click on the space and type. List the courses ONLY by their University ID (e.g. CHEM 301; PHYS 112 etc.)
3. If you have a class that goes for an hour and a half (e.g. 9:00 - 10:30), fill in both the 9:00 and 10:00 spaces
4. Save a copy of the form and then send it to .
