Schedule Forms for Teaching Assistants

Instructions for completing and submitting the Schedule Form from the Chemistry Computer Lab or from the Chemistry Software Server:

  1. To fill in all of your personal contact information and class schedule, click on the space and type. List the courses ONLY by their University ID (e.g. Chem 301; Phys 112 etc.)
  2. If you have a class that goes for an hour and a half (e.g. 9:00-10:30), fill in both the 9:00 and 10:00 spaces.
  3. Print this form to the PDFCreator printer to generate a PDF output file with all of your schedule information included on it. Print this file to the desktop and name it with your username.*
  4. Email a copy of the saved file to chemta@iastate.edu and print two copies of this form; one which you will turn in to Sally in 1608 Gilman and one for your records.

* It is extremely important that you print this form to the PDF printer so your schedule information gets saved in the output file. Since this is an electronic form, simply saving it will save only the form and not the data you fill in!

 

Alternate instructions for completing and submitting the Schedule Form using your own computer:

  2. If you have a class that goes for an hour and a half (e.g. 9:00-10:30), fill in both the 9:00 and 10:00 spaces.
  3. Print two copies of this form; one which you will turn in to Sally in 1608 Gilman and one for your records.
  4. Email a copy to chemta@iastate.edu
  • Click on the Submit by Email button and a box with three options will pop up. Choose Internet Email and click Ok. Another box will pop up that asks you to save the Data File; please save this to your Desktop. KEEP THIS BOX OPEN as you will need to follow the instructions to email.
    • Javascript must be enabled in your pdf viewer or the Email and Print buttons will not work!
  • Open Webmail, click Compose, and follow the instructions for emailing the data file to Sally as they are listed in the pop up box.
